Paperwork
Before we can schedule an Occupational Therapy, Physicial Therapy and Speech Therapy Evaluation appointment, we need a referral from your physician and your insurance information. Once you contact this office, we will send you a link to our Patient Portal to complete the remaining intake paperwork.
Your first appointment
We value the information that parents can provide during the evaluation. Parents are a part of the initial intake and are welcome to stay for the full Occupational Therapy, Physical Therapy and Speech Therapy Evaluation.
Your information
Any information shared with Capital Kids Therapies is HIPPA protected. You will receive a copy of our Privacy Practices included in the intake paperwork.
Text message poicy
On rare occasions, we may send a text message if we need to notify you of a schedule change and cannot reach you by phone. You will have the option of signing up for this service and opting out at any time. Message & Data Rates may Apply and Message frequency varies.
